As of March 2008, Hazlet Township is part of the Monmouth County Department of Health. The transition to the County Health Department will provide the citizens of Hazlet with increased services all at a substantial cost reduction.

Applicants are required to make an appointment to apply for a certified copy of a Vital Record. Appointments are also required to apply for a Marriage, Remarriage or Civil Union License.
Appointments will be taken on Mondays, Wednesdays and Thursdays from 8:00 a.m. to 4:30 p.m and on Tuesdays from 8:00 a.m. to 6:00 p.m.
