Hazlet Police Department Forms

All of the forms are fillable online and can be emailed directly to police headquarters or can be printed out and either delivered or faxed to police dispatch. 

The Temporary Handicap Permit and the New Jersey State Firearms Packet must be printed out and delivered to police headquarters.

Forms

  • Bicycle Registration Form (PDF) - This form will allow residents to inform the police department descriptions of their bicycle(s) including the serial number. The police department will supply the owner with a registration number for their records as well as being able to engrave the number on the bottom of the bicycle.
  • Business Emergency Contact List (PDF) - Use this form to provide emergency contact information and alarm information to police headquarters.
  • New Jersey State Firearms Packet (PDF) - These forms must be printed out and delivered to police headquarters. Packets can be dropped off anytime.
  • Preventing a Burglary (PDF) - Informational packet to help you secure your house/business the best you can.
  • Senior Citizen/Disabled Citizen Form (PDF) - Use this form to provide police headquarters information on special needs in case of emergency. This includes information on key holder or lock box information, emergency family or friend contact information, etc.
  • Temporary Handicap Permit (PDF) - This form is for a temporary handicap parking permit only. Not for the state issued handicap parking placard
  • Traffic Complaint Form (PDF) - This form is for residents who have traffic concerns in their neighborhood to notify the police department along with a description of the problem.
  • Vacation Notification Form (PDF) - This form is for residents who will be going on vacation or away for a designated period of time.
