Vital Statistics (Marriage & Death)
The Hazlet Township Office of Vital Statistics is responsible for documenting and maintaining records of marriages, civil unions, domestic partnerships and deaths that take place within the township. Forms are posted at the bottom of this page.
Marriage / Civil Union Application
The Marriage / Civil Union license application is to be made in the municipality in which either party resides and the license is valid throughout the State of New Jersey. If neither is a New Jersey resident, submit the application in the municipality where the marriage ceremony will be performed. The license is only valid in the issuing municipality.
Proof of identity for applicants by presenting government issues photo ID such as: driver's license, passport, or state / federal I.D.
Appointments are required to apply for a Marriage, Remarriage or Civil Union License. Appointment hours are Monday through Friday from 8:30 a.m. to 3:30 p.m. From Memorial Day to Labor Day, Friday's hours are 8:30 a.m. to 11:30 a.m. Please request an appointment.
Note: If you are divorced, have had a previous civil union dissolved, domestic partnership terminated or have had a civil union annulled, please bring the decree(s) or the civil annulment documents, if possible. The date and place of event is required.
Or if your former spouse / partner is deceased, please bring death certificate, if possible.
After you apply there is a 72 hour waiting period before the license is issued. The waiting period begins when the application is filed with the Local Registrar. Couples are advised to make application no later than two weeks prior to the wedding date.
Marriage / Civil Union / Domestic Partnership Certificates
Marriage / Civil Union / Domestic Partnership certificates must be obtained in the municipality where the event occurred, not in the municipality where the application was filed.

Hazlet Township handles death records. Applicants are required to make an appointment to apply for a certified copy of a Vital Record.
- Death certificates must be obtained in the municipality where the individual was pronounced dead.
- Proof of relation to the deceased must be provided.
- Fee: $10 per certified copy in the form of cash, check, or money order. No credit or debit cards.
