The granite monument was designed by Joseph Uras, a former Hazlet resident, and donated at cost. Donations were raised from the businesses and residents to fund the monument. It sits near the Veterans Memorial monuments in a landscaped area between the Cullen Center and Union Ave. in Veterans Park and was dedicated on Sept. 11, 2003.

The site is included in US Forest Service's National Registry of Living Memorials database of community-based responses to the tragic events on September 11th.
